-
Learning pathway: Level 3 Certificate in Social Prescribing with Need 2 Succeed (completed March 2025)
- Background: Former housing officer; transitioned to community health and wellbeing in 2023
Starting point & challenges
Priya entered the social prescribing space after over a decade in housing services, seeking a more holistic and preventative approach to supporting people. While passionate, she lacked formal training in health-based signposting, patient-centred practice, and managing complex caseloads in a primary care setting.
How Need 2 Succeed helped
-
Essential knowledge: The Level 3 Certificate provided essential knowledge on social determinants of health, motivational interviewing, and the wider landscape of integrated care systems.
-
Practical assignments: Assignments were deeply practical, allowing Priya to reflect on real patient cases and apply models of behaviour change, cultural competence, and asset-based community development.
- Tailored guidance: Tutors with lived experience in the sector offered tailored guidance, particularly around role boundaries and emotional resilience.
Results
-
Confidence and credibility: Priya now liaises directly with GPs and MDTs, presenting clear, structured care plans for patients.
-
Expanded toolkit: She introduced a local walking group and a peer-support network for isolated carers, both now embedded in her surgery’s referral offer.
- Recognition and progression: Her practice manager highlighted her in the ICS newsletter as “a model of best practice in cross-sector link work”.

“This course gave structure to everything I was doing instinctively. Now I don’t just connect people – I empower them to connect with their own communities, purpose and potential.”
Priya is now mentoring a junior colleague and exploring a Level 4 in Community Health and Wellbeing. She’s also been invited to sit on a local PCN working group shaping the future of social prescribing in North Central London.
Why Priya’s Story Matters
Priya’s journey showcases the power of structured professional development in supporting career transitions, especially into emerging roles like social prescribing. With the right training, experience from other sectors becomes an asset – not a barrier.
